Why See an Endodontist for Your Root Canal Treatment

By Dr. Shipra Jain, BDS | MDS · · Medically reviewed · Updated

Why Choose an Endodontist

An endodontist is a dentist who has completed additional specialist training focused on the inside of the tooth - the pulp, the root canal system and the tissues around the root tip. Where a general dentist treats the whole mouth, an endodontist spends their working life on root canal treatment and the problems that surround it.

That concentration matters because root canal anatomy is genuinely difficult. Canals are narrow, curved, and frequently branch. A molar may have three canals or five, and the extra one is often the one that causes trouble later if it is missed. Success depends on finding and cleaning every canal along its full length, and on sealing them so bacteria cannot return.

When to See an Endodontist

Many root canals are handled perfectly well by a general dentist. Referral is more likely to be worthwhile when:

  • The tooth has curved, narrow or unusually shaped canals
  • A previous root canal has not settled and needs retreatment
  • The tooth has a calcified canal that is hard to locate
  • There is a persistent infection at the root tip that may need endodontic surgery
  • The tooth has been fractured or knocked in an injury
  • Previous treatment left a separated instrument or a perforation to manage
  • Pain is difficult to localise and the source is unclear

Diagnosis is a large part of the job. Pain in an upper molar can come from a lower one, from the sinus, or from the jaw joint. Establishing which tooth - and whether the problem is endodontic at all - can save a healthy tooth from unnecessary treatment.

Endodontists Specialize in

Beyond training, the practical difference is usually equipment and time.

  • Magnification. A dental operating microscope makes visible what cannot be seen with the naked eye - the faint floor markings that indicate an extra canal, a hairline crack, the difference between a canal and a calcification.
  • Rubber dam isolation. A sheet that isolates the tooth, keeping saliva and its bacteria out of a canal system being disinfected, and preventing anything being swallowed. It is standard practice in endodontics rather than optional.
  • Focused imaging. Small-field 3D imaging can show root anatomy and the extent of infection in ways a standard X-ray cannot.
  • Time. An endodontist's day is built around these procedures, so difficult cases get the appointment length they need.

Reasons to Choose an Endodontist

The practical arguments for specialist treatment on a difficult tooth:

  • Retaining the tooth. Where the alternative is extraction, keeping a natural tooth is generally the better long-term outcome - and specialist treatment can make the difference on a tooth another dentist considers untreatable.
  • Fewer repeat treatments. Thorough cleaning and sealing the first time reduces the chance of a tooth needing retreatment later.
  • Comfort. Achieving profound anaesthesia in an acutely inflamed tooth takes technique. Endodontists do this daily, which matters if you are anxious or have had a previous appointment where the numbing did not hold.
  • Honest assessment. A specialist will also tell you when a tooth is not worth saving, which is worth as much as the treatment itself.

Summary

Root canal treatment has a reputation shaped mostly by the pain people arrive with, not the treatment itself. Done properly - full-length cleaning of every canal, sealed well, and restored with a crown afterwards - it is a routine way to keep a tooth that would otherwise be lost. Our guide to crowns after root canal treatment explains how the final restoration is chosen for different teeth.
