In a small number of cases, infection persists at the root tip even after excellent root canal treatment, or the canal anatomy makes conventional treatment impossible. Endodontic surgery - most commonly an apicoectomy - treats the problem directly at the root tip and saves the tooth.
As a specialist endodontic practice, we perform these microsurgical procedures in-house with magnification, precise instruments and gentle technique.

Endodontic surgery treats infection at the tip of a root when working through the crown of the tooth is not enough. The most common procedure is an apicoectomy: a small opening is made in the gum, the infected tip of the root and surrounding tissue are removed, and the end of the canal is sealed from below with a biocompatible material.
It sounds more daunting than it is. The procedure is done under local anaesthetic, usually takes under an hour, and is performed under the microscope - which matters, because the root tip is a few millimetres across and the accessory canals that cause the trouble are finer than a hair. Most patients are surprised by how ordinary the recovery is: a day or two of swelling, then normal life.
Surgery is not the first choice and we will not suggest it when retreatment through the tooth would work. It comes into its own when a canal is blocked by a post that cannot safely be removed, when retreatment has already been tried, or when a cyst has formed at the root tip and needs to be removed directly.

How long it takes
The procedure takes 45 to 90 minutes depending on which tooth and how much bone is involved. Stitches come out after about a week, and the bone continues healing quietly over the following months.
